SECTION 5 — HEALTH, SAFETY & CREATOR SUPPORT

Please share any health, allergy, medication, accessibility, behavioral, or support information that may help us provide a safe and positive experience for your Creator. Information provided will be used to support your Creator while participating in The Favor Lab Homeschool Community.

SECTION 6 — PERMISSIONS & AUTHORIZATIONS

Please review each permission carefully and select your preference. Certain permissions are required for participation, while others are optional.
Emergency Medical Authorization

In the event of an illness, injury, allergic reaction, or other medical emergency, I authorize The Favor Lab to contact emergency medical services and/or seek appropriate emergency medical care for my Creator when I cannot be reached. I understand that reasonable efforts will be made to contact me and/or the emergency contacts listed on this registration.

I understand that I am responsible for providing accurate and current health and emergency information and for notifying The Favor Lab of any changes.

Photo & Video Permission

Throughout the Creative Season, The Favor Lab may photograph or video Creators participating in activities, projects, showcases, special events, and community experiences. Photos and videos may be shared with participating families and may also be used on The Favor Lab’s website, social media pages, printed materials, advertisements, or other promotional materials.

A Creator’s full name will not be intentionally published with promotional photos or videos without additional parent/guardian permission.

Field Trips & Off-Site Experiences

The Favor Lab may coordinate optional field trips and off-site experiences throughout the Creative Season as opportunities for Creators and families to learn, explore, and connect outside of regular program days.

The Favor Lab does not provide transportation to or from field trips. A parent/guardian or other responsible adult must accompany their Creator and is responsible for transportation and supervision during the field trip.

Families are also responsible for any admission fees, tickets, meals, parking, or other costs associated with participation unless otherwise stated.

Field trip details, including the location, date, time, cost, registration deadline, and other requirements, will be provided to families in advance. Participation in field trips is optional.

Creator Participation & Activities

I understand that participation in The Favor Lab Homeschool Community may include hands-on art and craft activities, STEAM projects, games, movement, collaborative activities, use of age-appropriate tools and materials, and other creative and enrichment experiences.

The Favor Lab will use reasonable safety precautions and provide age-appropriate guidance during activities. I agree to provide accurate information regarding any restrictions, allergies, sensitivities, or other considerations that may affect my Creator’s ability to safely participate.

SECTION 7 — FAMILY AGREEMENTS & PROGRAM POLICIES

To help us maintain a safe, welcoming, and dependable community for all families, please review the following program expectations. Additional details will be provided in the Family Guide and during Family Orientation.
Attendance & Absences
Consistent attendance helps Creators build relationships and fully participate in the Creative Season. Parents/guardians should notify The Favor Lab as soon as possible when a Creator will be absent. Missed program days are not guaranteed to be rescheduled, credited, or refunded.

Health & Illness
Creators should remain home when they are ill, experiencing symptoms that may be contagious, or unable to comfortably participate in program activities. If a Creator becomes ill during program hours, a parent/guardian or authorized pickup person will be contacted for prompt pickup.

Community Expectations
Creators are expected to treat others with kindness and respect, follow safety instructions, respect the space and materials, and participate in ways that support our community. If ongoing behavior significantly affects the safety or well-being of the Creator or others, The Favor Lab will work with the family to determine appropriate next steps.

Arrival, Dismissal & Late Pickup
Creators should arrive and be picked up according to their registered schedule. Parents/guardians are responsible for arranging timely pickup by an authorized adult. Repeated or significantly late pickups may result in additional fees and/or a review of enrollment.

Tuition, Registration & Enrollment
The $75 annual registration fee is separate from tuition and is non-refundable once enrollment has been accepted and materials have been prepared or ordered. Tuition rates, payment due dates, withdrawal requirements, and other financial policies will be provided to families before enrollment is finalized.
