    This enrollment packet helps us get to know your child, prepare a safe and successful learning environment, and begin a strong partnership with your family.

    Most families complete this packet in 20–30 minutes. Please answer each question as thoroughly and honestly as possible. The information you provide is kept confidential and allows us to thoughtfully support your child throughout the school year.

      I understand that participation in Handpicked House involves activities that carry inherent and unavoidable risks. These activities may include, but are not limited to:

      • Indoor and outdoor play
      • Nature exploration
      • Walking on uneven terrain
      • Gardening
      • Climbing, balancing, running, jumping, and other gross motor activities
      • Use of playground equipment
      • Arts and crafts
      • Sensory activities
      • Cooking and food preparation
      • Interaction with animals and insects
      • Water play (when applicable)
      • Field trips
      • Transportation to and from activities by parents or guardians
      • Any other educational or recreational activity offered by Handpicked House

       

      I understand that these activities may result in slips, falls, cuts, bruises, insect bites, allergic reactions, exposure to environmental conditions, property damage, serious bodily injury, illness, permanent disability, or death.

       

      I understand that not all hazards can be anticipated or eliminated, even with reasonable supervision and safety procedures.

    • Assumption of Risk & Release of Liability

    • I understand that participation in Handpicked House involves inherent risks associated with children’s educational, recreational, and outdoor activities. These activities may include, but are not limited to, indoor and outdoor play, nature exploration, gardening, climbing, balancing, movement activities, arts and crafts, sensory experiences, field trips, and interaction with natural elements, animals, and other children. While Handpicked House takes reasonable precautions to provide a safe environment, accidents and injuries can occur.

      I voluntarily allow my child to participate in these activities and acknowledge that participation is at my own risk. To the fullest extent permitted by California law, I release and hold harmless Handpicked House, its owners, employees, independent contractors, and volunteers from claims arising out of my child’s participation, including claims based on ordinary negligence. This release does not apply to gross negligence, reckless conduct, or intentional misconduct.

      In the event of an emergency, I authorize Handpicked House to seek emergency medical treatment for my child if I cannot be reached. I understand that I am responsible for any resulting medical expenses.

      I certify that the health information I have provided is accurate and complete, and I agree to notify Handpicked House of any changes to my child’s medical condition, allergies, medications, or emergency contact information.

    • Throughout the school year, Handpicked House may photograph or record classroom activities, special events, field trips, and other program experiences. These images and videos help us celebrate our community, document learning, and occasionally share the joy of our programs with prospective families.Please indicate your permission below.*
    • Restroom Assistance Authorization 
    • At Handpicked House, we encourage children to use the restroom independently whenever possible while providing age-appropriate support as needed. Because some of our younger students are newly potty trained or are still developing independent toileting skills, staff may occasionally need to provide limited assistance.

      This assistance may include:

      • Providing verbal reminders and guidance
      • Helping with clothing fasteners (buttons, snaps, zippers, etc.)
      • Assisting with wiping when a child requests help or is unable to do so independently
      • Assisting with handwashing and hygieneHelping a child change into clean clothing after a toileting accident (using the child’s spare clothes)

      Handpicked House staff will always respect each child’s privacy and dignity.

      Whenever possible, children will be encouraged to complete toileting tasks independently, with staff providing only the level of assistance necessary.
