I acknowledge that I am receiving a tattoo from an apprentice and things may turn out different than the initial design as they are just starting out and mistakes are possible.*

I acknowledge by signing this agreement that I have been given the full opportunity to ask any and all questions which I might have about the obtaining of a tattoo, and that all of my questions have been answered to my full satisfaction. I specifically acknowledge I have been advised of the facts and matters set forth below and agree as follows:

If I have any condition that might affect the healing of this tattoo, I will inform my tattooer. I am not pregnant or nursing. I am not under the influence of alcohol or drugs.
I do not have medical or skin conditions such as, but not limited to: acne, scarring (Keloid), eczema, psoriasis, freckles, moles or sunburn in the area to be tattooed that may interfere with said tattoo. I don't have any infections or visible rashes anywhere on my body. I have advised my tattooer.
I acknowledge it is not reasonably possible for the representatives and employees of this tattoo studio to determine whether I might have an allergic reaction to the pigments or processes used for my tattoo, and I agree to accept the risk that such a reaction is possible.
I acknowledge that infection is always possible as a result of obtaining a tattoo, particularly in the event that I do not take proper care of my tattoo. I have received aftercare instructions and I agree to follow them while my tattoo is healing. I agree that any touch-up work needed, due to my own negligence, will be done at my own expense.
I realize that variations in color and design may exist between any tattoo as selected by me and as ultimately applied to my body. I understand that if my skin color is darker, the colors will not appear as bright as they do on lighter skin.
I understand that if I have any skin treatments, laser hair removal, plastic surgery or other skin altering procedures, it may result in adverse changes to my tattoo.
I acknowledge that a tattoo is a permanent change to my appearance and that no representations have been made to me as to the ability to later change or remove my tattoo. To my knowldege, I do not have any physical, mental or medical impairment or disability which might affect my well being as a direct or indirect result of my decision to have a tattoo.
I acknowledge I am over the age of 18 years old and I have truthfully represented that the obtaining of a tattoo is my choice alone. I consent to the application of the tattoo and to any actions or conduct of the repesentatives and employees of the tattoo shop necessary to perform the tattoo procedure.
I acknowledge the use of photography and/or videos for promotional purposes, including but not limited to posting on social media platforms, websites, and other marketing materials. I understand that these images may be used without further notification or compensation, and I waive any rights to inspect or approve the final product. I also acknowledge that my identity will be protected in these materials unless I give explicit permission otherwise.
To ensure the safety and security of both the artist and the client, the establishment is equipped with security cameras throughout the premises. Clients are required to wear appropriate clothing that adequately covers the areas being tattooed. For tattooing of intimate areas, it is mandatory that such areas remain covered at all times. The establishment will provide pasties or suitable alternatives to maintain your comfort and privacy during the procedure. If you have any questions or require clarification regarding appropriate attire, please do not hesitate to inquire. Your cooperation is appreciated.
I acknowledge the risks and concerns of using numbing cream for tattoos, including potential allergic reactions, skin irritation, or adverse effects due to improper application or prolonged use. I understand that while numbing creams can reduce discomfort during the tattooing process, they may also affect how the skin responds to the tattoo needle, possibly impacting ink absorption or healing. I recognize that every individual reacts differently to topical anesthetics, and that using these products is a personal choice made with informed consent and responsibility. I also understand that it is important to follow all safety guidelines, use reputable products, and consult with my tattoo artist or a healthcare professional if I have any concerns or pre-existing conditions.
