ADDITIONAL REGISTRATION DOCUMENTATION

The following documentation can be downloaded (highlighted text below) and completed by your healthcare provider. At the end of this section you will be given an opportunity to upload any documents needed. If your unable to do so now, you can download/print and email them to admin@kids-smart.com
All documentations must be updated/provided yearly. Children will not be accepted into our programs until the following documentation is received:

1) All registrations must include: State approved Immunization Records or Exemption Form.

2) A Child Health Statement form for those who are attending our ECE - Junior Smarties Camp.

3) All children needing on-site medications will need a Medication Release Form and medications stored in their original containers.

4) If your child needs emergency medications, a care plan is also required:

SUMMER SUNSCREEN AGREEMENT
Please review the following options and make your selection below:

Option 1:

PROVIDE MY OWN SUNSCREEN

I would like to provide my own sunscreen for my child(ren) that is at least SPF 30 and is continuous spray (for proper and safe application) for the entire duration of their time in the Kids SmART summer camp. I will provide at least one labeled bottle at the start of camp and will continue to replenish the supply as needed. I understand that if I fail to provide enough sunscreen for my child(ren) to have applied every 90minutes that they are outside my children’s safety is at risk (due to sun exposure and high risk of sunburn), and I may be required to pick my child up from camp until sunscreen is provided.

Option 2:

KIDS SMART PROVIDES SUNSCREEN - HIGHLY RECOMMENDED

I'd like Kids SmART to provide sunscreen with at least 45 SPF (brand and application may vary) for my children for summer camp. I agree to pay the one-time $70/child sunscreen fee that will cover the cost of this sunscreen. I understand that this fee is non-refundable and not prorated in any way, including based on attendance. Kids SmART sunscreen will be applied at least every 90 minutes that they are outside to all exposed areas. (CDHS-CCAP does not cover the cost for this optional fee. If selected, you agree to pay the amount listed amount)

5. Payment Policy: I agree to abide by the payment policies as set by Kids SmART, Inc. and will pay the proper dues IN ADVANCE of care. understand that parent fees are due by the 1st of the month if I am on CCAP, and that out of pocket fees are due prior to care. I understand that ifI do not pay and accrue a balance of $100, my child will not be allowed to attend Kids SmART until the balance is paid. Any delinquent balances may be sent to collections with the addition of late fees.

These photos or videos may be available for public viewing or used for promotional or advertising purposes. I understand that while in Kids SmART, my child's photo or video may be taken while participating in an activity. 10. Sunscreen Consent: Kids SmART, Inc. on occasion may find it necessary to assist or apply sunscreen to your child.
