By submitting this sponsorship form and completing the payment, you ("Sponsor") agree to the terms and conditions outlined by The She Rose Foundation USA ("Organization"). The Sponsor agrees to provide the selected sponsorship contribution, which will be used to support the Endometriosis Wellness Kit Drive and related initiatives. All contributions are non-refundable and must be submitted through the online payment system at the time of form completion. In return, the Organization will provide the Sponsor with the recognition and benefits specified within the selected sponsorship tier. The Sponsor grants the Organization permission to use its name, logo, and promotional materials for event-related marketing, including but not limited to social media, the event webpage, and printed materials. The Sponsor is responsible for providing high-quality logo files and other agreed promotional materials within three (3) days of submitting the form to ensure timely inclusion in promotional content. The Organization reserves the right to decline promotional content that conflicts with its mission or values. In the event of unforeseen circumstances, such as event cancellation or format changes, the Organization will notify the Sponsor promptly and discuss alternative recognition options. By submitting the form, the Sponsor acknowledges that they have read, understood, and agreed to the terms outlined and that this agreement represents the entire understanding between the Sponsor and the Organization regarding the sponsorship.*
