DAY CAMP REGISTRATION

All dogs must be booked and dropped off by 9:30am- NO WALK INS! initial*

We ask that all dogs attend camp AT LEAST once every three weeks. Dogs that have
not attended camp in 3 weeks or more, without notice, will be assessed as a new
camper and charged a $20 reevaluation fee. initial*

If your dog is going to be absent from day camp on a regularly scheduled day, you must cancel your reservation or notify The Academy by 9am that day. If you fail to inform The Academy of absences in a timely manner you will be billed in full for any scheduled days your dog did not show up. initial*

If your dog isn't feeling well (vomiting or diarrhea) please do not bring into daycare. Any dogs noticeably sick at daycare will be sent home  initial*

If your dog boards or attends camp at a different facility, please keep your dog home for at least 72 hours to help eliminate the spread of any viruses or contagious illnesses that may be present at other facilities.  initial*

I understand that The Ulti-Mutt Academy has the right to remove my dog from
daycare if they feel he or she is a risk to others. initial*

I hereby hold The Academy harmless of any liability for my dog or any of my dog's
actions including, but not limited to, intentional or unintentional actions
that cause harm or loss to a human being, animal, or property, during their
attendance at The Academy. initial*

I understand I will be notified of any medical problems my dog has. If my dog
experiences a medical emergency, I give my permission to The Ulti-Mutt Academy
to take my dog to the nearest Veterinarian even if they are unable to reach me
or my emergency contact. initial*

I certify that my dog is in good health and has not been ill with any communicable conditions within the past 15 days. My dog has not been exposed to Rabies, Distemper, Hepatitis, Leptospirosis, Parainfluenza, Parvovirus, or Bordetella (kennel cough) within a 30- day period prior to enrollment. I further certify that my dog is current on all vaccinations including Rabies, DHLPP and Bordetella. initial*

 I agree to provide proof of vaccinations prior to the start of day camp, and agree to keep my dog's vaccinations up-to-date during their future day camp attendance. initial*

 I agree to provide proof of Town License prior to the start of day camp, and agree to keep my dog's License up-to-date during their future day camp attendance. initial*

We ask that all dogs come to day camp with a collar or harness on. initial*

Camp packages must be paid upfront to receive a discounted rate. initial*

Payments can be made via cash, check, card, or online in Paw Partner by following the link in your emailed invoice. initial*

The Academy has the right to withdraw your dog's position in day camp due to scheduling and/or payment conflicts, and re-enrollment will be required for future attendance.initial*
