Business Participation Form for Sunday, September 28th from 1 - 7 pm (rain date October 5th - you must be available for both), Cedar Lane, Teaneck, NJ. NON-FOOD PARTICIPANT - WITHOUT A TENT PROVIDED $500NON-FOOD PARTICIPANT with Tent & Table Set-up - $800CRAFT FAIR VENDOR (only applicable to non-food businesses) $200FOOD VENDOR/RESTAURANT/CATERER WITHOUT A TENT PROVIDED $250 (You are required to bring and assemble your own table/chair/tent set-up in the spot assigned to you for the event). Additional permit fees may be required for health and fire and a (COI) Certificate of Liability Insurance will be required. Details to be provided once this form is completed. If your restaurant is located within the event space, you can set up outside your restaurant, all others will be assigned a spot.FOOD VENDOR/RESTAURANT/CATERER with Tent & Table Set-up - $500 Note: Additional permit fees may be required for health and fire and (COI) Certificate of Liability Insurance will be required. Details to be provided once this form is completed. A 10x10 tent and 2 6 ft tables will be provided at your assigned spot on event day. PLEASE NOTE THAT PAYMENT IS DUE IN FULL TO SECURE YOUR SPOT AT THE EVENT. EVERYONE IS PROVIDED WITH A 10x10 TENT AND 2 - 6 FOOT TABLES, EXCEPT FOR THE FOOD PARTICIPANT FOR $250 AND NONPROFIT PARTICIPANT.

DETAILED TERMS AND CONDITIONS WILL BE PROVIDED WITH INFORMATION ABOUT INSURANCE REQUIREMENTS, HEALTH PERMIT REQUIREMENTS, FIRE MARSHALL INSPECTIONS (FOR THOSE COOKING ON-SITE), ACCESS TO POWER AND MORE ABOUT THE EVENT.
