Alterations Form

This contract is made between Elizabeth Lee Bridal Alterations ("Elizabeth Lee") and the client listed below (“Client”), pertaining to the alterations of wedding attire, or other articles of clothing, along with any accessories as outlined below.

Please note: Additional measurements and fittings may be required to complete the alterations requested. In the event Client does not respond or miss any scheduled appointments to complete the alterations project there will be a delay in finish + pickup date. A rush fee may be required to complete the project in a timely manner as a result of the Client's delay.

Elizabeth Lee is only responsible for the alterations defined below and will not preform other alterations or adjustments that are not listed.

All alterations fees are to be PAID IN FULL at the time of signing this document, unless approved by manager. If any further alterations are agreed upon and invoiced and therefore not paid by due date, a $35.00 (thirty-five dollar) late fee will apply. **Any balances due are required to be paid within 30 days or at the next fitting, whichever date comes first.

All payments are non-refundable.

All items must be picked up within 14 days of being notified of being ready. If Client fails to pickup item(s)after the 14 days, a $15.00 per day storage fee will accrue. The account must be paid prior to pick up.

Please note: It has been recommended that the Client's item needs to be altered to prevent any structural malfunctioning, and the Client has choosen not to proceed with the recommended alteration(s) for the the structure integrity of the dress.
Elizabeth Lee is not responsible for any structural malfunctioning due to Client's choice not to proceed with the recommended alteration.

Post Alteration Changes:

In the event the alterations have been completed as agreed upon, and the Client changes their mind to alter the same task further, there will be an addtional charge to repeat the alteration service.

Example: Client chooses a hem length of item based upon a chosen shoe height and the item is hemmed accordingly as agreed. The Client changes the shoe height and wishes to redo said alteration.
