By submitting this registration form, the participant and their parent/guardian acknowledge and agree to the following:
Assumption of Risk: Participation in basketball tryouts involves physical activity and the risk of injury. The participant and their parent/guardian understand and accept these risks.
Medical Clearance: It is the responsibility of the participant and their parent/guardian to ensure that the participant is physically fit and has no medical conditions that would prevent them from participating in basketball activities. The participant should consult a physician if there is any doubt regarding their fitness.
Emergency Medical Treatment: In the event of an emergency, the parent/guardian authorizes the coaching staff and representatives to obtain medical treatment for the participant as deemed necessary.
Waiver of Liability: The participant and their parent/guardian release, discharge, and hold harmless Titans Basketball/Gem Basketball, its coaches, staff, and representatives from any and all claims, demands, or causes of action related to any injury or accident that may occur during the tryouts.
Photograph and Media Release: The participant and their parent/guardian grant Titans Basketball/Gem Basketball the right to use any photographs or video footage taken during the tryouts for promotional purposes.
Conduct and Behavior: The participant and their parent/guardian agree to abide by all rules and regulations set forth by the coaching staff and Titans Basketball/Gem Basketball, understanding that failure to comply may result in the participant’s removal from the tryouts.
