Thank you for your interest in being part of the Midwest Tri-State Massage Championship.

This event brings together massage therapists, wellness professionals, and industry leaders from across the region. Vendor and sponsorship opportunities are available for businesses looking to gain exposure and connect with attendees.

Please complete the form below. Submission does not guarantee placement. Approved applicants will receive confirmation and next steps.

Please note that lodging is not included.

Sponsor the refreshment station with a large logo of yours on the banner at the refreshment station.

Lounge Sponsor- $800
Sponsor the competitor or judges lounge with a large logo of yours on the banner outside of the sponsored lounge.

 

 

 

Terms, Conditions & Liability Waiver

By submitting this application, I acknowledge and agree that submission does not guarantee acceptance into the Midwest Tri-State Massage Championship. I understand that all vendors and sponsors are subject to approval and must adhere to all event rules, policies, and standards of professionalism.

I understand that all payments are non-refundable unless otherwise stated by event organizers. I acknowledge that vendor booths are not included in sponsorship packages and must be reserved separately if desired.

I grant permission for my business name, logo, and submitted materials to be used for promotional and marketing purposes related to this event.

I further understand and agree that participation in this event is voluntary and at my own risk. I hereby release and hold harmless the Midwest Tri-State Massage Championship, its organizers, staff, venue, and affiliates from any and all liability, claims, damages, losses, or expenses, including but not limited to personal injury, property damage, theft, or loss that may occur before, during, or after the event.

By submitting this form, I confirm that I have read, understand, and agree to all terms outlined above.

Photo & Media Release – Vendors & Sponsors

By participating as a vendor or sponsor in the Midwest Tri-State Massage Championship, I grant permission to the event organizers, affiliates, and representatives to capture and use photographs, video recordings, and other digital media of my booth, products, services, branding, team members, and overall presence at the event.

I also grant permission for any logos, images, or media I voluntarily provide to be used for promotional and marketing purposes. This includes, but is not limited to, use across social media platforms, websites, advertisements, printed materials, and future event promotions.

I understand that such media may be used without further notice or compensation. I waive any right to inspect or approve the finished content and release the Midwest Tri-State Massage Championship, its organizers, staff, and affiliates from any and all claims, demands, or liabilities related to the use of these images or materials.

By submitting this form and participating in the event, I acknowledge that I have read, understand, and agree to this release.
