In order to help us streamline the booking process please fill out any relevent information we may need for your appointment below. If there is a question you do not understand or are not sure how to answer, give a best guess and let us know at the end how we can help. Once this has been filled out you will be automatically put onto a list in the order your request was received, from there you will be contacted for booking and more information if needed. Tattoo Size - be specific. Tell us the actual size you are wanting, do not guess. 2” x 3”? 5’ x 8”? Use a ruler, or a common reference such as a baseball or dollar bill.*
