I have answered these questions honestly and to the best of my ability. I understand that if I am accepted for this trip, I am committing to be part of the team before, during, and after the trip. This includes attending meetings and finishing work assigned to me in order to help the trip succeed. I will make these a priority in my schedule.
During the time leading up to the trip, I will commit to give my best effort to live the life of a follower of Jesus including but not limited to: regular time with Jesus, moral behavior, showing grace to others, and living in community with other believers (including regular attendance at church and youth activities). I understand that the main purpose of this undertaking is to serve others. I am also willing to submit to the authority and leadership of the adults who are leading the trip.
I understand that if I don’t meet the responsibilities and expectations as laid out by the youth ministry leadership, I may be asked to leave the team with no refund. Trip attendees can be dismissed at the discretion of the youth minister.
I am enclosing a $100 deposit to show my commitment to going to Mexico. I understand that this deposit is non-refundable if I am accepted for the trip. (I understand that if my application is not accepted for the trip, my deposit will be refunded to me.) If I am accepted, I agree to pay the remaining $100 as it comes due. I also recognize that I am responsible for my fees associated with passport applications, immunizations, and spending money.
My parents are signing this too, as an acknowledgement of their support of me on this trip. (Married adults, please have your spouse sign instead.) They are committing to help make it possible for me to go in whatever ways they can. I will respect their decisions and show them appreciation for their support.
