I have answered these questions honestly and to the best of my ability. I understand that if I am accepted for this trip, I am committing to be part of the team before, during, and after the trip. This includes attending meetings and finishing work assigned to me in order to help the trip succeed. I will make these a priority in my schedule.

During the time leading up to the trip, I will commit to give my best effort to live the life of a follower of Jesus including but not limited to: regular time with Jesus, moral behavior, showing grace to others, and living in community with other believers (including regular attendance at church and youth activities). I understand that the main purpose of this undertaking is to serve others. I am also willing to submit to the authority and leadership of the adults who are leading the trip.

I understand that if I don’t meet the responsibilities and expectations as laid out by the youth ministry leadership, I may be asked to leave the team with no refund. Trip attendees can be dismissed at the discretion of the youth pastor.

My parents are signing this too, as an acknowledgement of their support of me on this trip. They are committing to help make it possible for me to go in whatever ways they can. I will respect their decisions and show them appreciation for their support.

As the parent/legal guardian, I have discussed this trip with my son/daughter and give him/her permission to be involved in this event. I also understand the above commitment that is promised by him/her and will do my best to help them honor it. I agree that the subject of this release will abide by the rules established by the youth ministry and the event. I understand that all reasonable safety precautions will be taken at all times by University Christian Church and its agents during the events and activities. I understand the possibility of unforeseen hazards and know the inherent possibility of risk. I agree not to hold University Christian Church, its leaders, employees, and volunteer staff liable for damages, losses, diseases, or injuries incurred by the subject of this form.
