As parent/legal guardian, I have reviewed the information about the CIY Move event, have discussed this trip with my son/daughter, and give him/her permission to be involved in the overall activities. I agree that the subject of this release will abide by the rules established by the youth ministry and the event. I acknowledge that if the subject of the release has to return home early for discipline violations, it will be at our expense. I understand that all reasonable safety precautions will be taken at all times by University Christian Church and its agents during the events and activities. I understand the possibility of unforeseen hazards and know the inherent possibility of risk. I agree not to hold University Christian Church, its leaders, employees, and volunteer staff liable for damages, losses, diseases, or injuries incurred by the subject of this form. I also understand the commitment that is promised by my son/daughter and will do my best to help them honor it.
