This agreement is between Ada Nsitem (“Therapist”) and the undersigned (“Client”).
Service – The Therapist will provide therapeutic support in sessions lasting 50 minutes for Individual Therapy sessions and 90 minutes for Group Therapy sessions (two or more individuals).
Platform – Sessions will be conducted via telephone call or other encrypted platforms such as Zoho Meeting, Google Meet, Signal or WhatsApp.
Confidentiality – All sessions are confidential except where the Therapist assesses a risk of harm to the Client or another person or if safeguarding a child or vulnerable adult is necessary.
Client Responsibility – The Client has the responsibility of ensuring they are in a private, confidential location where they will not be disturbed during sessions.
Technical Issues – If technical issues occur, both parties will attempt to rejoin. If the issue persists for 10 minutes or more, the session will be rescheduled to a mutually suitable time.
Cancellations – Cancellations must be made at least 12 hours in advance, after which the session will be rescheduled to the next available time. If cancellation is not made in this time, the session will not be rescheduled but the Client would be entitled to a partial refund, that is 80% of the full fee. However, there would be no refund if there were any applied discounts or reductions at the point of payment.
Payment – Payment confirms booking of a consultation/session.
Validity - Sessions must be conducted on a mutually agreed day and time slot within 12 weeks from the date of payment.
Teletherapy & Electronic Communication Consent
By signing this agreement, I (the Client) provide my explicit and informed consent to participate in teletherapy sessions via telephone, Zoho Meeting, Google Meet, Signal or WhatsApp and to the above terms and conditions of this therapy agreement. I understand that while the Therapist uses secure, encrypted platforms to support confidentiality, electronic communication may carry some inherent risks to privacy.
I agree to take responsibility for ensuring that my environment is private and that my device is secure during sessions.
By ticking the box, the Client agrees to the above terms.
