  • Safeguarding Incident & Accident Report

  • All accidents, incidents and near misses must be recorded on the online form below within 24 hours of the incident  / accident occurring, and ideally by the person dealing with the matter.  A ‘near miss’ is defined as an event or situation that could have resulted in a serious outcome but was avoided by chance or by intervention.

This information is reviewed by the Office for Safeguarding Children on behalf of the National Spiritual Assembly.

If a child needs urgent medical attention or is in immediate danger, call 999 without delay.

The answers submitted on this form may be read within a few days of the report being sent.  We do not currently have the resources to provide a 24 hour helpline nor can we provide professional advice to children, families, and volunteers.  When in doubt, please refer to the helplines at the end of this page and seek professional advice.  You can also contact the Office for Safeguarding Children at osc@bahai.org.uk.
