Safeguarding Incident Report

Please complete the questions below for any incident involving the safety of a child or group of children within 24 hours of the incident happening (the sooner the better).
The purpose of the report is to record and monitor any incidents on a national level, regardless of the outcome.

If a child needs urgent medical attention or is in immediate danger, call 999 immediately.

The answers submitted on this form may be read by several people appointed by the National Spiritual Assembly's safeguarding team within a few days of the incident being sent.

Please call National Office on 020 7584 2566 (office hours) if the matter is highly sensitive. We do not currently have the resources to provide a 24 hour helpline nor can we provide professional advice to children, families, and volunteers.
