Undergraduate Permit Request

This form is not for closed class permits. Before classes start, students need to join the waitlist. After classes begin, students need to reach out to the instructor for permission and forward approvals to their advisor or healthsciences@memphis.edu for a permit. Emails must include CRN, subject, number, and section.
