This form is to be used by person/company requesting permission to photograph and/or video any USA Powerlifting competition/event.

USA Powerlifting Media Policy

USA Powerllifting's Release of Claims, Liability and Indemnification Agreement contains releases from each participant, granting the right of USA Powerilfting to photograph and video the participants. We recommend you review the agreement to ensure it satisfies model release forms required for media production.

We are not accepting media requests for our National events as we have contracted photographers and videographers.

Please do not send Media Requests the week leading up to the competition as it might not be enough time for a response!!

To capture and depict only those images, videos, media, and photographs that show USA Powerlifting positively and professionally and that maintain its brand, public image, and good will.*
