Each volunteer (or their parent/legal guardian) must sign this form prior to event participation.
In accordance with the spirit of volunteerism and service, I assume full and complete responsibility for any injury or accident that may occur to me during my participation in this voluntary event, or while I am on the premises of any location related to this volunteer drive, including Galveston ISD facilities and Galveston Island Brewery facilities.
I hereby release and hold harmless United Way of Galveston, Galveston Independent School District, Galveston Island Brewery, other participants, and all sponsors, persons, and/or entities associated with Share Your School Supplies from liability for injuries and damages sustained to me, whether caused by negligence of the sponsors, other persons, or entities associated with this event, or otherwise.
I also grant GISD, UWG, and ABC13 (and their related parties) permission to use any photo or video footage that may contain my image captured while I am working on the Share Your School Supplies Drive project in conjunction with any of their publicity programs.
