PSC Swim Lesson Registration - Summer 2026

There are two ways to pay. You may send payment via Zelle to palisadesswimclub@gmail.com, or pay by check made out to Palisades Swim Club on or before the first day of your session.

1. ASSUMPTION OF RISK: I understand that participation in swim lessons involves physical activity in and around water and inherent risks including but not limited to drowning, injury, or illness. I voluntarily assume all such risks on behalf of my child. 2. RELEASE OF LIABILITY: I agree to release and hold harmless Palisades Swim Club (PSC), its staff, instructors, volunteers, and affiliates from any and all liability, claims, damages, or expenses arising from my child's participation in the swim lesson program. 3. MEDICAL AUTHORIZATION: In the event of an emergency, I authorize PSC staff to seek medical attention for my child. I confirm that all medical conditions have been disclosed on this form. 4. SUPERVISION POLICY: I understand that a parent or legal guardian must be present for the entire duration of each swim lesson. Failure to comply may result in removal from the program without refund. 5. PHOTO RELEASE: I grant PSC permission to photograph or record my child during lessons for program promotional purposes. If you do not consent, please note this in the Additional Notes field. 6. PROGRAM POLICIES: I understand that spots are limited, classes are first-come first-served, and advancement to the next level requires a successful exit skills assessment. 7. REFUND POLICY: I understand that registration fees are non-refundable. However, refunds may be considered on a case-by-case basis in the event of extenuating circumstances.
