Please complete the following form for up to 3 art pieces for the SALT charity art auction.SALT Outreach Inc. is a 501c3 non-profit organization (EIN#: 45-5360995) and all donations are tax deductible. Please include the value of the donated artwork(s) in order to receive a tax receipt. Notification - You will be notified via email if your art has been accepted for the art auction. That email will include important information such as drop off dates, times and locations.Liability - Please note: No insurance is provided for any artwork in the exhibit and is the sole responsibility of the artists. Submission of work constitutes an agreement in which the artist agrees to hold harmless SALT Outreach Inc.'s staff, volunteers and Board of Directors from any liability in connection with the works of art during the art auction.The artist gives permission to SALT Outreach Inc. to use photos of displayed artwork in any promotional materials.
