Terms and Conditions:
1. The Kibble Reward club will result in the 11th bag of food product being FREE
2. Only the type or size that is selected during signup is eligible.
3. The retail price of any item may change as a result of vendor price increases by manufacturer being passed on to clients.
4. If type or size of food is discontinued by the manufacturer then we will not be able to honor the agreement as the option no longer exist and is not replaceable
5. No replacement discount for other type or sized bags (or cans) of food
6. A separate agreement is needed for another pet, type, or size of food and it must be available at our hospital
7. Pet must be up-to-date on the prescription refill requirements below
· Prescription Refills must be approved by the doctor (includes prescription diets)
· It is based on the doctor's discretion of the pet's medical history and the situation.
· A doctor's exam has taken place within a year by any of our doctors. A legal requirement is a veterinary client patient relationship (VCPR). This is based on the status of Annual physical exam else the date of most recent medical examination.
· Testing requirements if needed has been completed (example: allergy screening)
· Anything outside the norm the doctor would need to approve in advance
8. The doctor must be consulted for approvals if switching diets or putting your pet on a new diet
9. Pets may share the same food if that medically and financially makes sense
10. One signup form is needed for prescription food option so essentially one per pet.
11. This program will be honored up to the 11th item before any discontinuation by Parkland Veterinary Hospital unless the manufacturer discontinues the product prior.
12. We cannot guarantee availability so clients are encouraged to call ahead to check availability.
13. Cannot combine with other related offers (for example Promo10 already gives a similar petfood discount.)
