I accept that decisions concerning the scholarship program may only be made by the VHA scholarship committee. I also agree that the scholarship funds will only be granted to me if I am enrolled as planned in a post-secondary educational institution in the 2026/2027 academic school year.

I recognize that VHA reserves the right to verify the information present on this application. If my enrollment is revoked for any reason, I will immediately notify VHA.

If you are a student who has taken post-secondary studies for at least one (1) year, please email a copy of your most recent unofficial/official transcript to hssp@vha.ca. The transcript must clearly show your name and your most recent cumulative GPA.

If you are a graduating high school student, please email to hssp@vha.ca either:

• The final report card for your first semester AND your most recent second semester report card

• The final report cards for your first, second and third semesters AND your most recent fourth semester report card if your secondary school is following a quadmester schedule.
