This tool assists in accurately recording medical notes and improving efficiency in patient communication. I understand that the ScribbleVet app will be recording audio of these interactions for the purpose of generating timely and accurate medical documentation. By initialing below, I acknowledge that I understand the purpose of ScribbleVet and consent to its use as part of my pet’s care experience.

I hereby authorize the Veterinarian to examine, prescribe for, or treat the above pet. I assume responsibility for all charges incurred in the care of the animal. I also understand that these charges will be paid at the time of release and that a deposit may be required for surgical treatment.
