The last day to complete this form is April 11, 2025. Note: The WCADAMHS Board is seeking submissions from artists living with mental health and/or substance use disorders and their loved ones. A decision on whether sales will be allowed will be made after submissions close, prior to the event, and artists will be alerted via their preferred communication method. If sales are permitted, this decision will apply only to approved submissions with a maximum of 10. Individuals will not be allowed to bring additional items for sale. The WCADAMHS Board will not handle payment transactions on behalf of any participants.
