    Adoption Application

    White Rock Dog Rescue
  • Thank you for your interest in a White Rock Dog Rescue pup!

We are a 501(c)3 non-profit rescue operating in the Dallas-Fort Worth metroplex. We are completely foster based (do not have a facility like a shelter, all our dogs are in foster homes) and are run completely by volunteers.

Completion of this form does not guarantee adoption.  We review all applications to find the best fitting match for the dog AND adopter.

Adoption fees are $350 for puppies under 1 years old and $300 for adult dogs over 1 years old.  Flat faced dogs (pugs, frenchies, etc.) are $425 for under 1 year and $375 for adults as they require a special vet for sterilization due to their flat faces/short noses.

The adoption fee includes all required vaccines, sterilization, and microchipping. Heartworm and flea preventatives are included with each vaccination visit.  If a service has not yet been completed, a voucher for the service is provided.

Adoption fees are due prior to physically taking the pup for a home trial visit. They are not collected on this form and are sent in a separate email request after meeting the pup.

Submitting this form is not a commitment to adopt, but rather it drives the whole process- setting up a meet and greet with the dog, getting any additional questions answered, etc.

  • The application typically takes fewer than ten minutes to complete. However, we request photos and videos of your property (the yard, the fenceline, and where the dog will sleep). Please complete this form while you are at home.

  • Applicant Information

  • Which age group do you fall into?*
  • Hey, young one! We don't mean to get preachy, but owning a dog is a big financial and time commitment. Can you confirm that you can provide a secure forever home for your new pet? Dogs can live 10+ years (and smaller dogs can live 15+ years) so it is a long term commitment. Another option would be to FOSTER a dog if you are not sure you are ready.*
  • As a foster, you would provide a rescue dog with a safe, loving environment until we find them a perfect forever home.

We provide our fosters with all necessary supplies, including food and a crate for the dog.

In return, we ask that our fosters assist with transportation to all required vetting appointments.

You'll get to meet a variety of dogs, have all your expenses paid, and not have to commit to full-time "parenthood" until you're ready.

  • Here's why we ask for your current driver's license:

    Verification of Identity & Address:
It helps confirm that applicants are who they say they are and that we have accurate contact information — especially important for adoption contracts and follow-ups.

Safety for the Rescue Team & Volunteers:
Many meet-and-greets or home visits happen at private homes or public spaces. Knowing who we’re meeting ensures everyone’s safety.

    Recordkeeping Purposes:
Adoptions are legal transfers of ownership, and we’re required to keep records. A valid ID ensures the paperwork is complete and compliant.

    Accountability & Animal Welfare:
Sadly, not everyone has good intentions. Checking IDs helps protect the dogs from ending up in unsafe or fraudulent situations.

Your privacy is important to us. We never share your information.

  • Household Information

  • I live in a ...*
  • If adopting a large or active dog, please confirm that you are committed to providing them adequate exercise.
  • My yard has a ______ fence.*

  • Current/Previous Pet Information

  • Have you ever owned a dog before?*
  • Are all of your current dogs sterilized and up to date on vaccines (proof will be required if your application is chosen)?*
  • Have you ever had to rehome (return, surrender) a pet?*
  • New Pet Information

  • No pet is perfect! Please tell us which behaviors YOU ARE NOT willing or able to work through (check all that apply or leave blank if you do not have restrictions):

  • Unless White Rock Dog Rescue (WRDR) otherwise specifies, puppies must be sterilized by six-months of age, and adult dogs sterilized within 60 days of adoption. Do you agree to abide by this timeline if you adopt?*
  • Puppies are not fully vaccinated and protected from illnesses (parvo, distempter, etc.) until 7 days after their THIRD ROUND of vaccinations. This means they need paws off the ground in public areas (NO DOG PARKS, NO SIDEWALKS, etc.). If you live in an apartment, you can use peepee pads or take your new puppy potty in areas where not a lot of other dogs walk. Do you agree to abide by these safety precautions?*
  • Virtual Home Visit

  • The last step of our screening process is a brief virtual home screen.

This isn’t about judging your home — we’re not looking for spotless floors or fancy furniture! It’s simply to:
🐾 Ensure safety — checking for things like secure fencing or potential hazards.
🐾 Find the best match — some dogs do better with a yard, others are happy in apartments.
🐾 Understand their environment — it helps us prepare them for their new life with you.

Please send a short video of your fenced yard capturing the entire perimeter of the fence. We just want to ensure the yard is secure so your new pup can't get loose!

Apartment dwellers may substitute a video of where the dog will get daily exercise (dog park, walking path) and porch/balcony if you have one.

  • Lastly, we would like to see where the dog will sleep- does not need to be exact bed or crate, just the general area, like the living room, bedroom, etc.

  • We appreciate your interest in adopting, but we are unable to match you with a rescue dog.

Based on our experience, we require all applicants to:

    • be the "head of the household"- own or be on the lease for the home.
    • agree to sterilize and vaccinate all their animals.
    • agree to a virtual home check. (No one from WRDR will physically visit your home, but we ask for videos and photos to ensure the dog's safety.)

Please reconsider your responses and fill out this form again if you meet the above requirements.

    Thank you,

    White Rock Dog Rescue
