    • If I experience any pain or discomfort during the session, I will immediately inform Kulab so that the technique may be adjusted to my level of comfort.
    • I give consent to Kulab Thao to perform, remove, and/or retouch lash extensions or any brow services on me.  
    • I understand & acknowledge any risks or complications associated with the procedure as they have been explained to me. (Irritation or Allergic Reaction)
    • I have been given the opportunity to ask questions regarding any risks, complications, or benefits associated with the procedure. 
    • I understand that there are occasions in which I may have an allergic reaction, irritation, or in rare occasions infection as a result of the application or removal of eyelash extensions. These reactions may result in needing to see a medical professional at my own expense.
    • I understand that the procedure may take 1 to 2 hours.
    • I understand results may last 6-8 weeks for brow lamination/lash lift & 2-3 weeks lash extensions.
    • I have received and understand that for optimal results and for sanitary practices I intend to follow the aftercare instructions provided to me and that a failure to follow these instructions may result in the loss or damage of my eyelashes.

    • I attest that I have given an accurate account of my medical/skincare history, including any allergies, prescription drugs, or skincare containing retinol, BHA, or AHA that I am currently taking/applying or intend to take.

    • I have read & agree to Kulab Thao policies.

  • Policy's Form

  •  Deposit Policy

    • All appointments requires a NON-REFUNDABLE deposit fee of $40.00 in order to secure & confirm appointment.

    Cancellation/Reschdeuling

    • If a cancellation or reschedule must be made, we politely ask that Kulab Thao must be notify 48 hours in advance before the date of appointment.

      If fail to do so, Kulab Thao, has the right to terminate the existing appointment & charge a 50% service fee due diligence.

    Tardiness/Late 

    • We understand that unforeseen circumstances may come up, we politely ask that the client must inform Kulab Thao immediately via text message or phone call.

      We allow a 10 minute minute grace period — if later than 10 minutes it will result in termination of appointment. Reschedule may require a new non-refundable deposit fee.

    Refund/Complimentary

    • If a refund must be made due to a serious circumstance, such as an allergic reaction, we politely ask you to notify Kulab Thao immediately — we allow 24 hour for the client to notify Kulab Thao. If fail to do so, the refund will not be made.

      Only 40% of refund will be made. Not including non-refundable deposit.

      If you happen to experience extreme lash extension fallouts after an appointment we ask you to notify Kulab Thao within 24 hours to receive a complimentary retouch. If fail to do so, retouch will be charged as fill price.
