[ ] If no Home-Owner's policy, a billing statement bearing AUTOMOBILE policy number.[ ] If you currently have a Home-Owner's policy, billing statement showing policy number.[ ] Home-owners Insurance DECLARATION Page (Your insurer can get this to you).[ ] Insurance Agent's website -or- Company Name, Agent Name, Tel, Fax and E-mail.

[ ] Deed of Transfer or Deed-in-Lieu for all surrendered homes in the last 4 years[ ] Quit-Claim Deed(s) filed in conjunction with any agreed surrender of a home
