RESPONSIBILITIES:
1. Coach agrees to maintain the ethics and standards of behavior set by the International Sports Sciences Association (“ISSA”) - Professional Standards of Personal Training (www.issatrainer.com/policies/professional-standards-of-personal-training/).
2. Client is responsible for creating and implementing his/her own physical, mental and emotional well-being, decisions, choices, actions and results. As such, the Client agrees that the Coach is not and will not be liable for any actions or inaction, or for any direct or indirect result of any services provided by the Coach.
3. Client understands coaching is not therapy and does not substitute for therapy if needed, and does not prevent, cure, or treat any mental disorder or medical disease.
4. Client understands that coaching is not to be used as a substitute for professional advice by legal, mental, medical or other qualified professionals and will seek independent professional guidance for such matters. If Client is currently under the care of a mental health professional, Coach will recommend that Client inform the mental health care provider.
5. Client agrees to communicate honestly, be open to feedback and assistance, and create the time and energy to participate fully in the program.
6. Client is making a commitment to his/her health and should follow the program and instructions of Coach to the best of their ability to maximize results and better achieve their goals. A fitness assessment and orientation is provided at no extra charge as part of the intake session.
7. Client understands that Wellness Coaching is a relationship between Coach and Client that is designed to facilitate the creation/development of personal, professional, and/or business goals and to develop and carry out a strategy/plan for achieving those goals. This is a comprehensive process that may involve all areas of life, including work, finances, health, relationships, education, and recreation. Client acknowledges that deciding how to handle these issues and implement choices is exclusively their responsibility.
8. Client understands that information will be held as confidential unless stated otherwise, in writing, except as required by law. Client understands that certain topics may be anonymously shared with other wellness coaching professionals for training or consultation purposes.
SPECIFICS:
Coach and Client shall agree upon the time, program type, content, and location of each session. Sessions will last approximately 60 minutes unless otherwise discussed. If Client anticipates running late, he/she should contact Coach as soon as possible.
CANCELLATIONS:
A 24-hour cancellation notice, by text or phone call, is required for rescheduling or cancelling any individual sessions. Client may cancel this agreement within 4 business days after the day the agreement was signed for a full refund of any monies paid toward this service. Once this time has lapsed, all fees are non-refundable.
Client may cancel this agreement if he/she relocates his/her residence farther than 60 miles away from the current location of the studio at 235 Main Street, Brandenburg, KY 40108.
Should Client become unable to use or receive services under this contract due to death or disability, Client, or Client's estate as the case may be, shall be liable only for that portion of the charges allocable to the time prior to the onset of such condition. Coach requires reasonable evidence of disability or death to be presented at the time of cancellation. Reasonable evidence includes, but is not limited to, a doctor's letter about the onset of disability or death certificate.
NO SHOWS:
Should Client not attend scheduled services without notice of cancellation, that service will be forfeit for that month and not allowed to reschedule.
SERVICES:
This agreement, between SERENITY FITNESS YOGA & REIKI, LLC and the above-named Client, will begin on date signed and will continue for the designated time from date of agreement or until terminated by Client or SERENITY FITNESS YOGA & REIKI, LLC (under the conditions stated in this agreement).
Coach will be available to Client by email and phone in between scheduled meetings as defined by the Coach. Follow-up Wellness Coaching sessions may be completed by phone or in-person. (This includes video messaging services).
PAYMENT:
Payment will be made in advance at the beginning of each month of service, over the course of the predetermined time as stated above. Payment must be completed for all sessions before any new or renewal contract or agreement may be signed. Coach accepts cash, Venmo, CashApp, or credit cards at this time. Payment is due no later than the 15th of each subsequent month. If Client's payment is more than 2 weeks overdue, Coach may suspend any remaining sessions until the outstanding invoice is current.
