Instructor Certification is required for a club/studio application. If you don't have instructor certification, you need to apply for instructor certification using the second option. Your instructor certification is subject to Regional director's approval. // Se requiere certificación de instructor para la solicitud de club/studio. Si no tiene certificcación, necesita utilizar la segunda opción para solicitarla. Está sujeta a la aprobación del Director Regional.

I understand that violation of the rules, regulations and by-laws may constitute grounds for revocation of my school license.
