This report is to be used when an incident occurs, including (but not limited to) material injuries, disciplinary events, accidents (e.g., slip-and-fall or car accidents while driving official WTSDA vehicles - rented or otherwise - or shuttling personnel during official WTSDA events), damage to facilities, etc.). It can also be used to report conflicts and complaints received as well as security-related issues. It should not be used for administrative-related complaints and does not need to be used for minor medical situations such as a person requesting a band-aid or other minor intervention for issues unrelated to the event. This report helps ensure we have a record of the incident and names of those involved in the event we need further information.
