Thank you for trusting YDE as a partner in your child’s education. This form is for all those who are seeking Tuition Assistance at YDE for the 2026-2027 school year, to be completed in addition to the TADS application.

Please be assured that your financial inquiry will be kept in the strictest confidence. YDE respects families of all financial means, and will do our best to provide a school of elevated academic, ethical, and religious enrichment for our community’s children of all financial backgrounds.

Families who have previously enrolled in tuition assistance must re-apply if they are seeking assistance this year.

Certification and Permission to Obtain Credit Information
We hereby affirm that the information contained herein and all documents submitted herewith are true and correct. I authorize YDE to request credit and/or asset information and/or to obtain copies of our tax returns (signed form 8821) in order to process or verify any information contained herein or in any of the documents submitted by us in connection with this application.

It is agreed that the grant of tuition assistance is conditioned on full payment, in a timely fashion, of the reduced balance due under the enrollment contract. YDE will have the right to reverse tuition assistance in the event of payment default.

In the event that any information submitted herewith is inaccurate, false or otherwise misleading in the opinion of YDE, or our financial condition improves in the future, we hereby agree to forfeit and return to YDE any and all moneys provided as financial assistance at any time, including past, present, and future financial assistance.

In the event that collection of these amounts becomes necessary, we will also be responsible to reimburse YDE for all costs of collection, including costs of investigation and reasonable attorneys’ fees.

We acknowledge that in return for Tuition Assistance, when called upon, we shall assist YDE at no charge, on an as-needed basis for School Function or Activity as determined by the School Administrator, but not more than a total of 20 hours per semester.

We give permission to YDE to obtain our credit report. We understand that this report will be held in the strictest confidence and will not be revealed to any other organization or agency under any circumstances whatsoever.
