YMCA Child Care Cancellation Form
We hope to provide services that meet you and your child's needs. However, we understand cancellations will occur. Parents/guardians must provide a two-week advance written request for refunds because of vacation, cancellation, schedule change, or account information change. YMCA Child Care does not provide refunds if your child is suspended for any reason. By submitting this form, you are providing written notice. Once your account has been adjusted, you will receive a confirmation email to follow up on your account.
