By submitting this form, I acknowledge the following (read and check ALL):*

I am 18 years of age or older.If my tattoo idea is accepted, I will pay the minimum $75 USD/$100 CAD (or $200 CAD for guest spots and conventions) non-refundable deposit within 2 weeks of deciding an appointment date. Deposits go toward the total of the tattoo. If I do not send a deposit, my appointment date may be taken up by someone else.There are no refunds for cancellations and no call/no show appointments, regardless of how far in advance Kera has been notified.I will give Kera AT LEAST 72 hours rescheduling notice or my deposit will be forfeit and cannot be put toward another appointment. Giving more than 72 hours notice will allow the deposit to be put toward a future appointment.I will give Kera AT LEAST 72 hours of notice before my appointment if I want minor changes to my design. Major changes may be cause for rescheduling my appointment or cancelling entirely.I acknowledge that if I change the subject matter of my tattoo or make major changes and give Kera less than a week of notice, my deposit may be forfeit and my appointment may be rescheduled or canceled.If my tattoo idea is accepted, UPON REQUEST, Kera can send a sketch as early as a week before my appointment or during a consultation. Kera will not show the finalized design until the day of the appointment.My tattoo idea may be declined for any reason.
