  • ATTENTION: This is an official vacating form. Please exit and submit once all lessees have vacated the apartment. 

  • Forwarding Address

    Leave us your forwarding address. Please note, this is for move-out statement purposes only, you must have your own mail forwarded via US Postal Services.  We do not do this for you. If you are on a joint lease your move-out statement will only be mailed to one address (the last submitted), with no exceptions.

    Please list the # of items you have left in your apartment. 

  • Contact Free Move Out

    1. Leave mail key, bedroom key (if applicable) and gate remotes in apartment.

    2. Once you have moved out lock your front door and dispose of the key. Call or text the office at (352)373-6330 to notify us you have completely vacated the apartment so we can change the front door lock.

  • If you plan on mailing your keys and gate remotes to the office mail them to:

  • I hereby certify that my apartment is vacant as of the date of my signature below. I understand that I may be charged for any damages that may be found by the management company above and beyond what my statement of apartment condition form indicates. Management may now enter the apartment at any time for the purpose of determining and correcting such damages which may include but not be limited to cleaning, painting, carpet cleaning/replacement, removal of personal property, garbage removal, maintenance and repair/replacement, and flea treatment. Charges may also include any remaining rental balances on my account.
