Welcome to Student Organization Petition (New Orgs Only) and Certification Form (Returning Orgs). By submitting the form below your organization and its undergraduate members will be certified with the Student Leadership and Engagement office and agree to abide by the tenants of the certification policy here: https://goo.gl/YwhsUY. Please note requesting this will allow your organization to be eligible for requesting funding from PSGA. The request must be sent via https://form.jotform.com/PSGA/funding_requests.

To complete this form you will need:

The name and contact information for 5 members of your organization (including student ID)

A person in the role of President, Vice President, and Treasurer (you can include the same people from your members list)

If you have more than 5 members, a list of your current members

The name and contact information for your advisor

A copy of your constitution
