Welcome to Student Organization Petition (New Orgs Only) and Certification Form (Returning Orgs). By submitting the form below your organization and its undergraduate members will be certified with the Student Leadership and Engagement office and agree to abide by the tenants of the certification policy here: https://goo.gl/YwhsUY.

Please note that submitting this form does not mean that you have been approved for funding allocations. You must sumbit this form to request general allocations as well as special funding: https://form.jotform.com/PSGA/funding_requests.

To complete this form you will need:
