By submitting the form below your organization and its undergraduate members will be certified with the Student Leadership and Engagement office and agree to abide by the tenants of the certification policy here: https://goo.gl/YwhsUY.

Please note that submitting this form does not mean that you have been approved for special funding requests. You must submit this form to request PSGA Special Funding: https://form.jotform.com/PSGA/funding_requests.

To complete this form you will need:

A person in the role of President, Vice President, and Treasurer (these can be the same as your certifying members)
