I hereby acknowledge that I am participating in a training class for dogs. I further acknowledge that such a class involves an element of risk in both the on leash and off leash exercises. Therefore, I absolve the instructor, assistants, and owners of the premises as well as other participants in the class of responsibility or liability for any accidents of injuries that may result from said activities. Pictures/videos taken during the class are the property of Carol Upton (Carol the Dog Trainer) and may be used for promotional material.*

I acknowledge the above and agree to continue with the class.my dog's vaccinations are up to date and in accordance with Ontario laws. For young puppies, a minimum of 1 set of vaccinations is required but as we are working outside, 2 sets is preferred. Please check with your veterinarian.I confirm that my dog has NO history of biting or aggression towards other dogs or people OR I have fully disclosed my dog's bite history. I will muzzle my dog if/when required by a city order or as requested by the class instructor/participants.I acknowledge that some classes may be held outdoors in public spaces.If your dog has significant fear-based aggression/reactivity toward strangers or dogs, or is extremely uncomfortable in new environments, group classes may not be the best place to start. We reserve the right to remove a dog from class if the situation seems detrimental/unsafe for that dog or other dogs/people in class, and transfer you to private sessions.I will comply with all city regulations for dogs in public locations.

In case of inclement weather, outdoor classes will be postponed or held on Zoom. Remember to check your email for an update about 2 hrs prior to your class start time.Westboro outdoor location: Broadview Ave Public School parking lot (the one at Avondale Ave intersection)Westboro indoor location: Champlain Park Field HouseGlebe indoor classes are held at the community center (GNAG CC) at 3rd and Lyon.A minimum of 3 registrations are required to run a class with a maximum of 6 per instructor.Class length is 45 min. and 60 min for accelerated curriculum.Missed classes are non-refundable but you can make-up any missed class by attending one in another ongoing/subsequent session at any location for up to 3 months after your original class start date.I have read the above!
