Memento Ornaments Order Form

Available in sets of three 3"x3" or sets of two 4"x4" $60 per set
- Fill out the contact information.
- Upload a high-resolution jpg, png, and tiff with a minimum resolution of 300dpi.
- Enter the custom label text into the form. Finally, select the sizes and quantities for each finish option (black or birch).
  To order Memento Photo Blocks return to TheMementoMaker.com/mementos-order and select Order Photo Blocks.

Image Eligibility

Images must be copyright free or you must hold the copyright.

Printing Rights are not the right to manipulate the image.

Photographers hold the copyright to their images and may provide printing rights to a client/third party. This is not the right to change or manipulate the original image.

You may contact your photographer to request permission for the image to be turned into an encaustic photograph. The photographer must have filled out and electronically sent me the permissions form.
