  • This form is for Girl Scouts in the GSNYPENN Pathways council serving parts of New York State and Pennsylvania. If this is not your council, do not submit this form. Contact your own council for instructions.

    This form is to be used after completing your pre-approved community service hours.

    If you did not request pre-approval from GSNYPENN for your community service, please do so now by clicking here. Then, wait until you receive approval to submit this form. Please note that requesting retroactive approval does not guarantee it will be granted. 

    Instructions:

    • Make sure you have the correct form. This is for the Community Service Bar. (The Service To Girl Scouting Bar final summary report is here.)
    • Submit this form for completed community service at your pre-approved organization within one year of receiving council pre-approval.
    • Attach your verified log of service.
    • Allow up to 10 business days for council to review your final summary.
    • Once your final summary is approved, you and your troop leader/adult volunteer will receive a congratulatory email.
    • After you receive notification that you have earned the award, the Community Service Bar pin may be purchased from our council retail store. Our staff can confirm your award status.

    • Please locate your pre-approval email, confirm the date you received pre-approval, and enter this information in the date box.

  • Log of Hours:

    Please upload a typed document that includes the following:

    1. Your name (The Girl Scout)
    2. Your troop number or Juliette status
    3. The organization's name where you volunteered
    4. The contact person's name and phone number
    5. The contact's signature/verification/acknowledgment (this can be typed)
    6. The tasks you completed with dates and number of hours (you must have completed a minimum of 20 hours)

    Hour log options:

    1. An email or letter from the contact on the organization's letterhead stating the above information will be accepted as hours verification. Please attach it to this submission. (A high-quality photo is acceptable.) 
    2. You may also type your information into a Word or Excel document. However, the contact will still need to verify it. Please ask them to sign and date your log, then attach it to this submission. (A high-quality photo is acceptable.)
