Grievance Form: Tournament & Junior Team Tennis (JTT)

Please submit this form if you would like to file a grievance for one of the following: adult tournament, junior tournament, or junior team tennis.

USTA Mid-Atlantic expects the highest standards of conduct at approved events.

The USTA Mid-Atlantic Grievances & Sportsmanship Committee is charged with reviewing all allegations of unsportsmanlike conduct or violations of regulation submitted within a grievance. Prior to submitting a grievance, please ensure you have reviewed the USTA Mid-Atlantic Grievance Procedures. Grievance information is not confidential and will be shared with the Committee and all parties involved with the grievance.

Note: Video submissions of any kind are not permissable in the USTA grievance or appeal process. All information submitted pertaining to a grievance or grievance appeal must be in writing in order to be shared with the Committee and all parties involved. All files and attachments uploaded to this grievance form must be in PDF format. Please make sure that all information is included as your grievance form will be considered complete upon submission.

All players, parents, and spectators must accept and sign the Mid-Atlantic ACES Pledge. If you haven’t done so already, you must sign the ACES Pledge immediately. No action will be taken if the individual filing the complaint has not signed the ACES Pledge.
