If you would like a replacement, then this is not the correct form for you.
We have two different options for a warranty replacement.
The first option is to have a return label emailed to you to send back the incorrect switch. Once we receive the return we will send out your replacement. To select this option the correct form you need is here: Label via Email for Warranty.
The second option is to place a penny deposit down and have the replacement switch shipped immediately with a return label. As long as you return the item with the enclosed return label within 21 days you will not be charged again. To choose this quicker option the correct form you need is here: Penny Deposit for Warranty.
