How this form works
The Owner/Pilot, Authorised WOF Inspector, and Admin will automatically receive a PDF copy of their application.

Each step of the signature process will be actioned automatically, when the required email fields are filled, once the applicant submits the form.

This form is a controlled document, managed by the NZHGPA, under the delegation of the Civil Aviation Authority (CAA).
Form Instructions
1. Complete all areas where applicable;

2. Paragliders must pass this inspection every 12 months, or every 100 hrs, or at the manufacturer's recommended interval, whichever comes first;

3. There are two options for completing this form;

i. Owner/Pilot Checks: checks on recreational gliders may be carried out by the OWNER/PILOT, with minimum PG2 certificate, under supervision of an authorised WOF Inspector.

ii. Authorised WOF Inspector Checks: checks can be done soley by the Authorised WOF Inspector;

* Porosity Test Instructions:

If test 1 (top surface, centre above B lines) is < 60 seconds, a further 4 tests must be carried out (2 top, 2 bottom surface) across span, and if possible, different colour panels.

Authorised WOF Inspector:

If test is < 60 seconds:

Porosity testing is NOT required for gliders < 12 months old or 100 hrs flight time, whichever comes first;

If porosity is <60 sec, or the Authorised WOF Inspector has any doubts as to the airworthiness of this glider, they shall refer to the glider manufacturer and NOT sign this form until they are certain the glider meets manufacturer guidelines.
