*With the exeption of the NBME Authorization Form and Residency/Licensing/Graduation Verification forms, forms that have/require a statement concerning "Good Standing" or requires enrollment verification must be submitted using the Good Standing, Enrollment Verification, & Graduation Verification Request Form.

Certification of ID and Authorization Form:

To gain access to the NBME Certification of Identification and Authorization form, apply for the test on their website.

Please be sure to complete the authorization portion of the form before submitting for signature.

AAMC Standardized Immunization Form:

1) All Sentry MD requirements must be "Compliant" or "In-Progress"

2) Learner Name and Birthdate must be at the top on all four (4) pages of the AAMC Standardized Immunization Form
