I agree that I will not sell, give away, or otherwise dispose of said animal to any person(s), dealer, retailer, auction, institute, shelter, rescue organization, or any other entity for any reason. If at a later date I am unable to keep this pet, I agree to first contact RRR of South Louisiana @ 225-788-6940 or rrrofshouthla@aol.com. RRR of South LA is an in-home foster program. We do not have a facility. I agree that if I am unable to keep this pet, I will transfer ownership back to RRR of South LA and foster the animal in my home until a new home can be found.*

I/We agree to activate the 30 days of FREE pet insurance being provided. This insurance must be activated as soon as possible, no later than within 24 hours of adoption. Activation of this free insurance is mandatory. Please note that every effort has been made to ensure that your pet is in good health when arriving at an adoption event. However, any time an animal is moved to a new environment, it does cause stress. When an animal is under stress, it may cause minor digestive, skin, or respiratory upset. If any symptoms appear, please contact your personal vet. RRR of South LA is not responsible for any medical expenses incurred after the adoption of said pet.*

I/We agree that this pet is not to be altered by tail docking, ear cropping, or declawing. Agree* Until your pet has received a complete series of vaccinations, your pet should be kept away from all other animals that are not up to date on their vaccinations, as well as, away from public areas where other dogs may be (i.e. Parks, Petco, PetSmart).*

I hereby agree to care for the above-described pet in a humane and responsible manner and to provide it with clean and adequate shelter, food, water, and veterinary care (including but not limited to yearly vaccinations). I further agree that said pet shall reside inside my home and shall not be allowed to roam freely.*

If not already done, it is mandatory to spay or neuter your pet using the spay/neuter voucher supplied to you here today. Failure to do so within 90 (or number of days*) days will result in the surrender of this pet back to the RRR of South LA.*

I/We understand that RRR of South LA makes no representations or warranties, expressed or implied, regarding the health, temperament, size, or breed of this pet. I/We agree to be solely responsible for this pet, as well as, indemnify and hold harmless RRR of South LA from any and all claims of liability for the conduct of this pet on or after the date of this adoption. This release of liability and indemnification shall apply to all known, unknown, and unanticipated damages resulting from my/our adoption, ownership, or control of this pet.*
