  • PARENTAL & YOUTH PROMISE OF SUPPORT

  • The leadership team of the "Our Lady of the Annunciation - Federation of North American Explorers" (hereafter called the Group) will strive to do their personal best to deliver to our youth a progressive, constructive and exciting program. Your child’s program experiences will not only be fun, different, exciting, and more, but will also serve as a positive and ongoing "education for life" experience.


    In order for the leadership team to be able to deliver such a program, behind the scenes support from you, the parent(s) is critical. Each family will be asked to indicate how they can help support efforts to ensure a dynamic youth program experience for each and every youth in our Explorer family.


    Besides parental support, your child will also be required to make a promise of commitment to the Group and the program. This commitment includes:

    • Wearing proudly the full and complete uniform as designated by the leadership team
    • Doing his or her best to live up to the Promise, Law, and Group Code of Conduct
    • Attending all camps (fall, winter, spring, summer)
    • Participating to the best of his or her ability in all group meetings, outings, and fundraising projects.


    As parents/guardians, we ask that you review and support the following statement through your signature:

    "We as parent(s)/guardian(s), along with our son or daughter, will do our personal best to support the "Our Lady of the Annunciation-FNE" Group while also ensuring that our son or daughter actively participates in all of the various program activities including various group fundraising activities. We also understand that non-support for the Group or poor program participation on the part of our son or daughter could result in his or her program membership spot being made available to another child."

  • PHOTOGRAPH, FILM, AND VOCAL RECORDING RELEASE

  • I authorize this release based on the following conditions:

    1. These records become the property of the Our Lady of the Annunciation FNE Group.
    2. This release is given without promise of compensation.
    3. This is effective until terminated by a retraction in writing from the person granting the authorization.
    4. The undersigned does release to the Our Lady of the Annunciation FNE Group any right, title and/orinterest of any kind they may have in the records produced.

    I hereby grant to the Our Lady of the Annunciation FNE Group the right and authority to photograph, film and/or record vocally myself and my child(ren).
    These records may be used for promotional or publicity purposes and may be published in mass mediapublications, in Our Lady of the Annunciation FNE Group newsletters and other FNE publications, on the Our Lady of the Annunciation FNE Group internet sites and other FNE internet sites, or shown on television or movie presentations. The undersigned’s name and the first name and last initial of mychild(ren) may be used. This release is effective until revoked in writing by the undersigned. Such revocation shall only be effective to prevent any expanded future use of the records.

  • MEDICAL CONSENT

  • For all Our Lady of the Annunciation FNE Group activities, unless previously revoked in writing, I/We hereby give the official in charge, or his appointee, permission to arrange for medical or surgical attention for my child(ren) / ward(s) in the event of an emergency where I and other family members cannot be contacted in a timely manner. I understand that I will be notified as soon as possible should this authority be exercised.

     

    In the event of an emergency, I hereby grant permission to any staff to obtain medical care from a licensed physician, hospital, or medical clinic for my child(ren) in the event I cannot be reached.

  • I hereby grant permission to any staff person to provide the following over-the-counter medication to my child(ren) if requested by my child.

  • WAIVER OF LIABILITY

  • The undersigned do hereby release, forever discharge and agree to hold harmless Our Lady of the Annunciation FNE, the Diocese of Wheeling-Charleston, the Canons Regular of the New Jerusalem, St. James Catholic Church, and the Federation of North-American Explorers from and against any and all liability, claims, demands, lawsuits and expenses of any kind arising from personal injury, sickness, death or property damage of any kind whatsoever which may be incurred or suffered by the undersigned and/or the undersigned’s child(ren) / ward(s).

    The undersigned further agree to indemnify and hold the Our Lady of the Annunciation FNE, the Diocese of Wheeling-Charleston, the Canons Regular of the New Jerusalem, St. James Catholic Church, and the Federation of North-American Explorers, and their respective clergy, members, directors, officers, employees and agents (collectively, the “Indemnities”) harmless from and against any and all liability, claims, demands, actions, and causes of action whatsoever arising out of or related to any loss, damage, or injury, including death, that may be sustained by anyone, or to any property belonging to anyone, while participating in such activity, while in, on or upon the premises where the activities are being conducted, regardless of whether such loss is caused by the negligence of the releasees and/or participant, or otherwise and regardless of whether such liability arises in tort, contract, strict liability, or otherwise, to the fullest extent allowed by law.

  • Uniform Order

  • If you need uniform pieces, please fill out the Uniform Order Form separately for each person that has uniform needs. Any questions about uniforms can be directed to Lauren Marta at themartas@gmail.com

  • Registration Fees

  • Due to a decrease in donations and fundraising events, and the increase of costs of uniforms and camps, Registration Fees have been increased this year. We encourage each family to prayerfully consider making a full payment, or a payment plan that includes installments for these fees. Nevertheless, we remain committed to offering an affordable program for all families. No child should be prevented from Exploring due to financial reasons (see details in the Scholarship section). Fees will be due on the second meeting in October. Please make your checks payable to: “Our Lady of the Annunciation FNE”.   •  *Uniform rental allows the child to receive the full uniform for a new member, and replacement for outgrown parts for returning members. This fee does not cover the full cost of the uniform, but rather spreads out the cost in a three year cycle that includes shipping costs and additional uniform parts that need to be traded in once outgrown. Please note that all uniforms, and all materials with the FNE logos are property of FNE and must be returned once no longer needed. 

    ** Camp fees cover 3 weekend camps in the year. This amount does NOT cover the summer camp. As such, parental involvement in fundraising will be necessary to raise the funds to sufficiently offset the costs of summer camps.

    ***Wayfarer uniform fees only apply for those new and not serving in a unit. Our group will cover the cost of National fees and uniform for uniformed leaders.  Current wayfarers already in uniform,will only pay national fees to remain active (unless serving in a unit). 

    OLA-FNE will cover National Fees and Uniform Rental for uniformed leaders serving in a unit.

    Large families finding these costs to be prohibitive can pay the Total Fees for the first three oldest kids, plus $40 dollars for each additional child (National Fees).  The rest of these fees will have to be covered by fundraising and donations.

    For all families experiencing financial strain, please request a partial or full scholarship. We ask you to contribute as much as you can, but if in need of assistance, please talk to Father Alban, CRNJ at crotnj@gmail.com

  • Volunteering Point System

  • The Our Lady of the Annunciation FNE Group, like all FNE groups, is an all-volunteer movement. We cannot deliver a high quality program to your children without your help! We urge all parents to prayerfully consider how they can best support the Group. Please indicate below how you can best support your sons and daughters within our FNE family. If both parents are willing to volunteer please check the "Add second parent" box below.

  • Service Requirements

    Our program is available to our families thanks to the dedication and thousands of hours of hard work of our uniformed chiefs, assistants, and volunteers, We ask each family to contribute time and talents to the development and improvement of everyone’s FNE experience. This year we will be introducing a point system of volunteer hours to spread out the work and responsibilities in all the member families.  Each role and volunteer participation will be assigned a number of points, Each family will have to contribute a minimum of 10 points of service. We are always in need of uniformed volunteers as well as committee chairs, chaperones, and coordinators for specific works of mercy and fundraising events. More details will be found in our new FNE Family Handbook and explained at the Parent Meeting.
