PMH-C Renewal Form
Use this form to submit your Continuing Education Credits for the PMH-C. This form is exclusive to be used after you have completed the PMH-C exam, and received your certification credentials. This form should not be used as part of your PMH-C application, and is not a replacement for the PMH-C application.6 Credits per year or 12 Credits for two years, submitted every 2 years.CE submissions are reviewed and approved monthly. After the review, you will hear back from PSI regarding your status (approval, follow-up). If you have multiple documents, please combine the documents into a single file.
