    We are asking for the assistance of jail and prison staff, public defenders, Criminal Justice liaisons, etc., in submitting completed Prospective Resident Applications, because we are a small staff and are receiving applications from across the United States. We want to be fair in choosing the men whom we feel that we can best meet their mental, physical, emotional and legal needs and help them succeed. We cannot make "knowledgeable decisions on approving or denying an individual without having all the information needed and signed by the appropriate individuals. It is crucial that we receive pages 13 and 14 completed and signed by the individuals asked for. We really need your help on submitting COMPLETED applications, because we are trying to work as a team member with your agency to help men have a smooth transition from incarceration, addiction, etc. to success in the "real world".

    Our process is that the prospective resident completes the application in its entirety. Page 13 is the Medical/Mental Health Summary which must be completed and signed by the medical or mental health staff. Page 14 requires that either the re-entry specialist, IPPO or other appropriate representative (public defenders, Criminal Justice Liaisons, etc.,) attach the most recent 24-month disciplinary record (if applicable).

    Furthermore, at the bottom of the page there is a question that states "who will be paying the $475 entrance fee?" This section must be completed before submission. If all pages (including Pages 13 and 14) are not completed and signed by the approprlate indfv[dual(s),the application will not be reviewed,

    Once we receive the application and all processing is complete, we will send the application to our Reviewer and the Director of Project Operation Excel who will set up a phone call on a given Tuesday with the appropriate staff person and the applicant to make sure all the rules and other issues are understood. Once approved, an acceptance letter is sent to the individual and he will be placed on the waiting list. When a bed comes available, we will notify the IPPO or other appropriate Individual (Public defender. Case Manager, etc.) and ask that a Home Plan be filed. We will await a release date and then make arrangements of whether TDOC or a jail facility is sending them to us by bus or transport or a family member will be picking them up and transporting the individual.

    Dear Prospective Resident:

    Thank you for your interest in Aphesis House. Please read the "Aphesis House Prospective Resident Application Process", because you are responsible for completing and signing all appropriate pages which require your signature and insuring that the pages requiring medical and/or mental health history and staff signatures are complete. Plus, if the Prospective Resident Application does not contain all of the required documents and appropriate signatures, your application cannot he reviewed. Furthermore, failure to disclose your complete criminal background or other falsehoods on your application can result in an automatic denial of residency.

    A $475.00 Money Order for our Admission Fee is required upon arrival at our facility. WE DO NOT ACCEPT RHP. The Admission Fee includes the payment of your first two weeks at one of our facilities, a 31-day bus pass and other charges which are directly related to assisting our residents, plus administrative cost. The "Program Overview" page covers the Admission Fee more in-depth. After the first two (2) weeks, a program fee of $150 per week is required.

    Do not send in the Admission Fee until you report to the appropriate Aphesis House facility. Please read and re-read the Program Contract thoroughly, so you understand our rules and requirements to make sure that can abide by them before submitting the Prospective Resident Application.

    Our 90-day program consists of a structured environment primarily for men coming out of incarceration or drug treatment and is approved by the Tennessee Department of Corrections as a transitional living facility. Aphesis House, Inc. (AHI) requires four (4) in-house meetings a week (Anger Management, Relapse Prevention, Critical Thinking, and Spiritual Foundations) and one outside meeting(s) (either a 12-Step Recovery meeting, a local church worship service, or both, as dictated by the treatment team. Each resident is responsible for chores each day and the curfew is 6:00 p.m. or the first 30 days and 9:00 p.m. for the remainder of the program. Furthermore, each resident is drug and alcohol tested when he enters AHI, returns from a weekend pass (no overnight passes for the first 30 days), and on a random basis each month or if a staff member deems that the resident may be under the influence of an intoxicant (drug or alcohol). AHI has a zero tolerance policy regarding alcohol and illicit drugs. Those individuals with a co­ occurring disorder (addiction and mental health disorder diagnosis) or other medical condition that requires medication must be medication compliant and mentally, emotionally and medically stable.

    Our residents are provided an assessment of individual needs, orientation to the AHI rules and regulations, case management services, medical assistance, mental health referrals (when needed), individual counseling, etc. and the staff works with them to gain employment and succeed through an intensive project entitled Operation Excel which provides the resident with job readiness, life skills, budgeting and saving 75% of his net pay in a special account to prepare him to gain permanent housing upon leaving AHI. Our Admission Fee is $475, which covers two weeks of program fees, a 31-day bus pass, birth certificate, state ID, food until the resident is signed up for .Food Stamps, some needed cloth, work boots, etc.---whatever it takes to help the resident prepare for success.
