2024 Spring Registration Form

 

STEAM Learning Club invites you to enter the student and parent's information for registering public speech and debate programs. This form also provides general instruction and policy information. The information you entered will ONLY be used for records and class communication purpose. If there is any question, please contact steamlearningclub@gmail.com

IMPORTANT:

• Our school is located at: Suite 1401, 1503 & 1602, 8751 Collin McKinney Pkwy, McKinney, TX 75070. 
• If you are registering more than one student, please fill out this form for each student. We recommend each family use ONE email account for all communication, including class invitation, scheduling and progress update, etc.
• Please note below fee applies to the class sessions from Jan 7, 2024 to May 25, 2024, excluding Spring Break (March 10, 2024 to March 16, 2024) and makeup class week (May 19, 2024 to May 25, 2024), 18 weeks in total. All class schedules are in the Central time zone.
• The class groups may be adjusted or merged to have a minimum of 4 students. The listed class schedule may change due to teachers' availability or meet most students' availability.
• In extreme weather, we may cancel or reschedule the class, or covert the in person class to online class.
• If students cannot join the group classes because of personal reasons (such as sickness, time conflicts and other interventions), the missed classes are not refundable, neither can be credited. We will schedule makeup classes if you notus in advance but avaialibity cannot be guaranteed.
• Please check terms and services, refund policy before completing registration.

REGISTRATION & REFUND POLICY:  

• New student registration fee and materials fee are non-refundable.
• Class fee will be fully refunded with withdrawal notice before Dec 15, 2023.
• After Dec 15, 2023, 100% of the remaining fee can be credited for other programs or siblings within the current semester, OR 80% of the remaining class fee can be credited towards to the next semester, OR 50% of the remaining class fee can be refunded.

EARLY BIRD DISCOUNT (For Full Semester Payment Only):

• 10% off if register and pay by 11/30/2023
• 5% off if register and pay by 12/15/2023
• Addtional 5% off for siblings if register and pay by 12/31/2023
• Addtional 5% off if register 4 or more programs by 12/31/2023
• No discount if not register for the full semester - 18 sessions.

Safety Liability Waivers and Release of Claims Consideration:

 

Safety Liability Waiver 

I acknowledge the personal benefits accruing to my child by reason of participation in STEAM Learning Club activities and classes, and am aware of the activities which my child will be involved through said participation. Assumption of Risks: I am aware of the risks associated with participation in the above event and do hereby voluntarily assume full responsibility for any risk of loss, property damage or personal injury, including death, that may result from participation in event activities.   

 

COVID-19 Risk Acknowledgment and Liability Waiver 

• I acknowledge the contagious nature of the Coronavirus / COVID-19 and that the CDC and many other public health authorities still recommend practicing social distancing.  

• I further acknowledge that STEAM Learning Club has put in place preventative measures to reduce the spread of the Coronavirus / COVID-19. 

• I further acknowledge that STEAM Learning Club cannot guarantee that I will not become infected with the Coronavirus / Covid-19. I understand that the risk of becoming exposed to and / or infected by the Coronavirus / COVID-19 may result from the actions, omissions, or negligence of myself and others, including, but not limited to, STEAM Learning Club staff, volunteers, and other students and their families. 

• I voluntarily signed up for services provided by STEAM Learning Club and acknowledge that I am increasing my risk to exposure to the Coronavirus / COVID-19. I acknowledge that I must comply with all set procedures to reduce the spread while attending my child attends after school activities.   

 

Release of Claims Consideration 

Release / Indemnification: I hereby, in consideration of such benefits and other good and valuable consideration received, consent to the above listed participation and release absolutely, forever discharge, hold harmless and covenant not to sue STEAM Learning Club, and school location, its directors, employees, agents, volunteers, and affiliates from any and all present or future liability, claims, demands, actions or rights of action, whether asserted by me or a third party arising out of my child's participation in the event activities (the "Claims"). I agree to indemnify and hold harmless STEAM Learning Club and school location for any such Claims brought by me or a third party from any costs associated with defending or litigating such claims, including but not limited to attorney fees, costs and legal expenses.
