Students will be studying a media-rich curriculum, which will often include the use of video and photography. It is therefore inevitable that student images will feature in student work. In addition, we'd love to share our teachers' and students' experiences so that our school's approach to innovative education can benefit more students in the future.
To this end, we may occasionally have to use student imagery in promotional materials, which will include general promotion, social media, and on our website. We do not propose sharing these images with third parties, and we will take the necessary care to protect the privacy of our students and families. Students will similarly be taught how to respect each other's privacy as part of our focus on media literacy and persistent online identities. In order for this work to be done, we request the explicit consent of our parents that these images may be used for the purposes stated above.
I, hereby grant permission to Saint Too Bloom Academy to take photographs and/or digital images of my child / children for use in media releases and/or educational materials. Images may be used in printed publications or materials such as brochures or posters, electronic publications (such as email newsletters) or social media, or on Saint Too Bloom Academy's website.

Agreement and Authorization for Medical Treatment

醫療協議和授權同意書

If I, or the aforementioned person, cannot be contacted, Saint Too Bloom Academy is authorized to send my child/children to the Accident and Emergency Department of a government hospital for any necessary medical treatment on my behalf. I agree to my son/daughter receiving emergency medical treatment as considered necessary by the medical authorities present. If application of anesthetic and/or surgical operation is required, Saint Too Bloom Academy is authorized to sign all necessary documents and I will be responsible for expenses incurred. Saint Too Bloom Academy staff will continue to reach the parents and guardians of the child/children, until contact has been made.

I acknowledge the need for responsible behavior on the part of my son/daughter, and I will inform Saint Too Bloom Academy immediately of any changes in medical conditions or circumstances. I understand that Saint Too Bloom Academy will not be responsible for any consequences of medical treatment.

如學校未能即時聯繫本人，聖道百卉書院獲授權送本人的孩子到鄰近的政府醫院接受任何緊急治療。我同意我的孩子接受在場醫護人員認為必要的緊急治療。如果需要進行麻醉和/或外科手術，聖道百卉書院有權簽署所有必要的文件，本人將承擔所需的費用。聖道百卉書院的教職人員將繼續聯繫孩子的父母或監護人，直至成功聯絡其中一人。

本人明白我需要為我的孩子負責任，如本人的孩子有任何醫療狀況上的任何變化，我會立即通知聖道百卉書院。我了解聖道百卉書院將不對任何醫療後果負責。

If the Birth Certificate is not from Hong Kong, please upload copies of:

Enrolment 入學條款

Upon submission of the Saint Too Bloom Academy Online Enrolment Form, your child is officially enrolled in Bloom for the corresponding school year.

一旦遞交此入學表格，你的孩子將正式在相應學年註冊為聖道百卉書院的學生。

1. Parents are liable to a non-refundable payment of one-month tuition which will serve as the first instalment of the enrolled school year. 家長需繳交一期學費為留位費，留位費將作為學生正式加入聖道百卉書院後的第一期學費，不設退款。
2. The annual school fee for the 2024/25 academic year is HKD 196,680 for Grade 7-8, payable over 11 instalments at HKD 17,880 per instalment (pending EDB approval). There might be slight adjustments to our tuition fee at the beginning of each academic year, all fee disparities will be settled upon the second instalment of the enrolled school year. 2024/25 學年 7-8 年級年度學費為港幣 196,680，每年分 11 期收取, 每期為港幣 17,880（有待教育局審批）。請留意每年學費或有調整，所有費用差異將延至第二期學費處理。
3. A non-refundable annual miscellaneous fee of HK$4,000 will be charged at the beginning of each academic year. 家長每年需繳交港幣 4,000 作年度雜項開支不設退款。

Withdrawal 退學條款

If your child is going to withdraw from school, please notify the school via email at admissions@bloom.edu.hk. 

如你的孩子需辦理退學手續，請發電郵至 admissions@bloom.edu.hk 通知學校。

1. Please notify the school in writing at least 60 days before the last attendance date. 請於最後上課日的 60 天前以書面形式通知學校。 
2. Should it be an urgent withdrawal effective immediately, parents are liable to full tuition for the month of withdrawal regardless of the number of school days attended that month. Parents will also be required to pay tuition for 2 extra months due to the notice period policy as stated above. 如你需要辦理緊急退學手續，遞交退學申請該月份的學費不設退回。根據以上 60 天通知的條例，家長需另外繳交 2 期學費。
3. The annual miscellaneous fee is not to be refunded upon withdrawal. 年度雜項開支在任何情況下不設退回。

Financial Aid Assistance 學費資助條款

If your child receives financial support from Saint Too Bloom Academy, there is an expectation that they will remain at Saint Too Bloom Academy until graduation from Grade 12. Apart from family circumstances requiring the family’s relocation from Hong Kong, if your child leaves prior to graduation, the family may be asked to repay the financial aid fees granted during all of their time here with us.

Financial aid recipients are obliged to inform Saint Too Bloom Academy immediately should their family’s financial situation improve. We also reserve the right to review the financial status of each recipient family periodically to determine the eligibility for on-going financial support.

獲學費資助的學生需在聖道百卉書院完成整個中學階段直至 12 年級畢業。除因特殊狀況促使家庭搬離香港以外，學生若在畢業前離開，家庭可能會被要求歸還全數學費資助金額。

此學費資助的金額在本學年持續生效。我們將於每個新學年開始前，根據你提供我們的新文件和最新家庭收入狀況，修訂來年經濟援助的金額。

年度修訂取決於該年度收入證明、納稅申報表和所有其他相關證明文件的審核。如果你的財政或收入情況有變，請儘早通知我們。

如果您的財務狀況有變而未有申報，聖道百卉書院保留要求償還費用差異的權利。

Declaration

I hereby declare that we are fully disclosing all relevant information concerning our child's/ children's special or additional learning needs. We understand that any intentional omission of information may result in their expulsion from the school. Should there be any changes to our child's/ children's learning needs, the school will be notified immediately.
