I, the parent/guardian of the below named applicant, intending to be legally bound, hereby give my/his/her consent to that applicant to participate in CJ Brewer's 3rd Annual Dreamers and Believers Youth Football Camp to be held on June 20th, 2025, at Bowdon High School - 504 W College St., Bowdon, GA 30108 (the "Event" In recognition of, and with knowledge of, the fact that engaging in the sport of football and a football clinic/camp, are, or can be, inherently dangerous activities and involves the potential risk of personal injury, I voluntarily warrant that the applicant is in good physical condition and hereby agree to assume the risk of any injury that the below named applicant may suffer as a result of my/his/her participation at the Event. I further recognize that Participation/attendance in the Event is voluntary for the participant.
I, the parent/guardian and on behalf of my/his or her company, his or her heirs, executors, and administrators, hereby release from any and all liability, agree not to sue and forever discharge CJ Brewer, and all individuals and organizations associated with the Event, including but is not limited to each of their respective representatives, Bowdon High School employees, volunteers, subcontractors, sponsors, licensees, agents, affiliates, subsidiaries, parents, owners, officers, directors, volunteers, shareholders, the city of Bowdon, GA and lessees of the premises (collectively the "Releasees") from any and all liability, any and all claims, demands, actions, causes of action or suits arising out of any injuries, known or unknown, which have resulted or may in the future result from participation in the Event or use of any equipment, as well as from all present and future claims, liabilities, obligations, damages, losses, demands, costs or expenses (collectively, "Claims") for property damage, illness, personal injury,or wrongful death arising as a result of participation in the Event and related activities. I further acknowledge that food and beverages provided at the event are donated items that may have been manufactured in a facility containing nuts, gluten and other ingredients that may cause an allergic reaction and if the below named participant has any allergies, I, the parent/guardian, will bring lunch, snacks and beverages, which shall only be shared with the participant named below. If participant does not suffer from any allergies, I, the parent/guardian, recognize that no outside food or drinks, with the exception of water, are allowed to be brought to the event.
Please check box if participant requires medication or their own lunch due to dietary restrictions/allergies and requires you, the parent/guardian, to have a medical backpack on site at the event.
In the event of an emergency, I also give my consent for EMT to obtain, through a physician, EMT, trainer on site or local hospital, medical care as is reasonably necessary for the welfare of the participant. Additionally, I acknowledge that I currently carry medical insurance for the participant, and I give my consent for the participant to receive emergency medical treatment in the event of injury and illness and agree to be responsible for all costs associated with my/his/her transportation and treatment. I consent to administration of first aid and other medical treatment in the event of injury or illness.
I, the parent/guardian, affirmatively swear that, I am, at the time of signing, of legal age and fully competent to and do hereby execute this Release and Waiver on behalf of myself, my heirs, and the below named applicant. I further represent and warrant that I have read and fully understand the terms of this document and their legal significance.
Any photos/videos taken during the Event may be used for promotion by CJ Brewer's 3rd Annual Dreamers and Believers Youth Football Camp & its representatives. No compensation, monetary or otherwise, shall be granted for the use of any photo containing camp attendees or participants. It is agreed that without any further consideration from the Releasees, to grant the Releasees the unlimited right throughout the world to use my name, nickname, image, likeness, voice, photograph, signature facsimile, and biographical information in any matter and for any purpose in any media now known or hereafter existing in perpetuity.
This release is freely and voluntarily executed by the undersigned and not as a result of any inducements, promises or representations made by any party.
